Rachael Gavin

Rachael has a career in cardiovascular care that spans 25 years. Having worked in several roles at Lincoln County Hospital, including Clinical Practice Facilitator, she joined Lincolnshire Community Health Services in 2005 as Heart Failure Community Matron/Complex Case Manager, holding this position until 2013. She spent a further seven years as a Heart Failure Specialist Nurse, before moving into her current role of Senior Advanced Nurse Practitioner, managing proactive and reactive healthcare for over 500 care home residents.

With a commitment to education, Rachael has sought to develop and extend her nursing skills to an advanced level, most recently completing an MSc Health and Social Care (Palliative and End of Life Care). Following this, she led a collaborative action research project within the local Heart Failure MDT and currently holds the position of guest lecturer on the MSc Health and Social Care at Nottingham University, delivering sessions on Symptom Management in Advanced Heart Failure.

Rachael has been an Education for Health associate since 2018, supporting the Level 6 and Level 7 Heart Failure courses as a tutor and marker. She is delighted to be part of the Masterclass series this summer.
