This course is designed for health care professionals who encounter the challenges of managing patients diagnosed with chronic heart failure, in particular in the community setting. This exciting distance learning course provides a combination of self directed study, on-line learning, practical case scenarios and interactive study days. The course is ideal for qualified staff who deals with long term conditions, and need advanced practical skills and expertise in the assessment, investigation and treatment of patients with chronic heart failure. The course provides in-depth knowledge on the epidemiology, pathophysiology, investigation, diagnosis, pharmacological and non-pharmacological management including specialist referral and palliative care. At the end of the course practitioners will be equipped to deliver advanced models of care for patients with suspected or an existing diagnosis of chronic heart failure. The course is in line with best practice and Skills for Health (CHD GB2 / HWB64 / PSL 5 / HWB2 (3), and equips the students to lead in the implementation of the new NICE chronic health failure guidelines 2010/1.
