This module is aimed at practitioners working in a team leader (leadership) role. It addresses the broader multi-disciplinary aspects of service delivery which influence the heart of professional practice and strategic planning. It will equip students with the knowledge and skills to analyse critically effective chronic disease management, concentrating on the application of theory to their own practice in real life situations. Study at this level will enable the student to evaluate the service they provide, thereby benefitting people living with chronic disease.
