This course is aimed at health professionals working in clinical practice.  It will equip students with the knowledge and skills to underpin effective chronic disease management, concentrating on the application of theory to their own practice in real life situations. Study at this level will enable the student to make a difference to the service they provide, thereby benefitting people living with chronic disease.
